Report an incident

StaffStaff appRole permission: medical.incidents.manage

File the report while the detail is fresh. Reach it from Incident on the Home tab.

  1. Tap Child and pick the child. You can search by name or classroom.
  2. Choose a Type: Injury, Illness, Behavioral, Property Damage or Other. It starts on Injury.
  3. Choose a Severity: Minor, Moderate, Serious or Critical. It starts on Minor.
  4. Write the Description. This is required.
  5. Add Location and Action Taken if they help. Both are optional, and both are worth filling in — they are the two things parents and licensing ask about most.
  6. Tap Submit Report.

You see Incident report submitted successfully and the form clears, ready for the next one.

The description is read later by a parent, your facility admin and possibly a licensing inspector.

  • Facts, in order, with times: what led up to it, what happened, what you did.
  • Name only the child the report is about. Describe other children by role, not by name.
  • Action Taken should say who provided care and whether anyone else was informed.

The report is submitted to your facility immediately — it does not sit as a draft. Your admin reviews it in the web app, and parents can see and acknowledge it in the parent app.

  • There is no time field. The report is stamped with the moment you submit it. If the incident happened earlier, say so in the description, with the actual time.
  • You cannot attach a photo here. If photographs matter, take them through the Photos screen and tell your admin they relate to the report.
  • There is no notify-parents option. Notification is handled by your facility, not by this screen. Follow your facility’s rule about telling a parent in person or by phone — do not assume the app has done it.
  • There is no draft and no edit. Read it once before you submit. If something is wrong afterwards, tell your admin.

An incident report is a record, not a response. Deal with the child, call emergency services and tell your facility lead first. File the report once the child is safe.
