Incident reports

ParentsParent appRole permission: medical.incidents.manage

Go to the More tab and tap Incidents to see Incident Reports for your children, newest first.

Each entry in the list shows the type — Injury, Illness, Behavioral, Property Damage or Other — a severity of Minor, Moderate, Serious or Critical, the description, the date, and its state: Draft, Submitted, Reviewed or Closed.

Tap one to read it in full. Depending on what staff recorded, you may also see Location, Action Taken and Notes.

When a report is Submitted and you have not acknowledged it yet, an Acknowledge button appears at the bottom. One tap records that you have seen it, and the report then shows Acknowledged on with the date.

Acknowledging means “I have read this”. It is not agreement with what happened and it does not close anything off. If you have questions or you disagree with the account, acknowledge it so your facility knows it reached you, then raise it with them directly.

  • The app is not the place for an urgent conversation. Phone the facility.
  • Staff file reports for small things as well as serious ones — a graze is recorded the same way — so the presence of a report is not by itself cause for alarm. The severity is the signal.
  • If an incident needed medical attention, ask for the paper record too. Facilities keep licensing records beyond what the app shows.
  • Reports appear only after staff submit them, which may be some hours after the event. You should still have been told in person or by phone about anything significant.
  • You cannot edit or comment on a report in the app, and you cannot delete one.
  • No incident reports means nothing has been filed for your children.
